Office Hours: 7 days a week from 7 a.m. to 4:00 p.m.
Instructions on Obtaining Medical Records Information Needed: Patient Name, Identification, Date of Birth, and Phone Number of Requester. A signed release must be received before records can be forwarded to someone other than the patient.
Cost for Medical Records:
A $10 search fee is charged for third party requests in accordance with W.Va. law plus 40 cents per page. There are no charges if records are forwarded to another physician or hospital. |
